What is a Certified Community Health Worker? 

A Certified Community Health Worker (CCHW) assists patients to remove barriers to healthcare, social needs, and health education. Our CCHW is trained through the Kentucky Office of Community Health Workers, the Kentucky Association of Community Health Workers, and the National Association of Community Health Workers.

Assistance Opportunities

Our CCHW will assist with the following:

  • Access to care (physicians, hospitals, and clinics)
  • Medical related health insurance issues.
  • Assistance with Medicaid, housing applications, job applications, medical transportation scheduling, and more!
  • Referrals to partnering agencies for additional services such as: counseling, harm reduction/ syringe access and exchange program, health education, nutrition education, WIC (Women, Infant, and Children), and HANDS (Health Access Nurturing Development Services).
