We are looking for a Health Services Coordinator to join our team.

The individual will promote health and wellbeing by developing and implementing programs that educate individuals, families and communities about health topics, while also advocating for policies and resources that support healthy lifestyles.

  • Plan, direct and coordinate staff from multiple departments.
  • Assess health needs within the community. Create and implement health education programs, materials and events to address needs.
  • Educate groups on disease prevention, healthy behaviors, and accessing healthcare services. Evaluate program effectiveness. Collect and analyze data to improve programs.
  • Advocate for policies and resources that promote health.
  • Coordinate with the management team on all plan reviews, including Strategic Plan, Quality Improvement Plan, Performance Management Plan, and Workforce Development Plan. Maintain documentation for all Quality Improvement projects.
  • Responsible for finding funding opportunities for the organization and writing polished proposals to earn grant money.
  • Oversee the public health accreditation process/annual report and reaccreditation.
  • Administer the development and implementation of the community health assessment and community health improvement plan through the Mason County Coalition.
  • Coordinate the Mason County Health Coalition and participate in the subgroups.

Minimum Education, Training and Experience Requirements:  A Bachelor’s Degree, from an accredited college or university in Business or Public Administration, Health Administration, Public or Community Health, Social Science, Nursing, Education, or other related degree.

Five (5) years of experience in an administrative or management capacity where health related planning responsibilities have been demonstrated.


We are accepting applications for a Full Time Health Educator I.

General Duties include:  This position will serve under the direction of the Health Services Coordinator.  Responsibilities for this position include but are not limited to:

  • Meet with members of the community to establish a working relationship and become familiar with available community resources. 
  • Assist in exchanging information with various organizations and agencies concerning health needs of the community and assist in assembling materials to be utilized. 
  • Gather health information for the public relative to the specialized program. 
  • Distribute health education materials by a variety of methods. 
  • Participate as a team member, having a basic understanding of the team process. 
  • Demonstrate an awareness of public service announcements (PSA) and assist in writing public service announcements for radio and television. 
  • Submit written material to supervisor for approval. 
  • Determine working contacts with radio, TV and newspaper personnel. 
  • Collect and analyze data for health program needs.
  • Other duties as assigned/needed.

Buffalo Trace District Health Department is accepting resumes for a Community Health Worker.

Duties and Responsibilities:

  • Successfully complete a training program and practicum in home visiting services.
  • Conduct home visits, perform assessments of client and family needs for information about mental health, health and social services, and provide information and education about services, provider referral information to clients and their families.
  • Act as a liaison between clients and their families, mental health and health and human service providers, and report to network members the specific characteristics of conditions, which impede clients from obtaining available services.
  • Work with multi-disciplinary teams to establish action plans for clients and families; assure action plans are carried out, link clients with all needed services; connect clients to support groups; provide emotional and educational support for clients and their families.
  • Maintain precise records in accordance with protocol defined in the Community Health Workers manual.
  • Participate in continuing education programs, in-service education programs, and peer support system with other Community Health Workers.
  • Support research by recruiting and enrolling difficult to reach clients into study groups in accordance with IRB research protocols.
  • Administer study measures, collect, and enter research data in accordance with IRB research protocols. 
  • Work with HARM Reduction Specialist and its department’s clients.

Qualification for this position:  High School Diploma or equivalent

                                                    Organizational and communication skills  

                                                    Experience in Social/Mental Health
